Story is a wee bit misleading, I would imagine the main culprit was the removal of the cat leading to an exhaust leak.

Misleading in what way? That's still a modification gone wrong.

Reminds me of when the exhaust manifold used to crack on the E53 X5s

How on earth did they die from it though? Surely they'd smell the fumes and pull over? Or at worst start feeling faint and pull over!?

To be fair I didn't read the specific article, I read the story elsewhere where the author's focus seemed to be on the bonnet vents.

Carbon monoxide poisoning is not a grand deal different to hypoxia, where you really don't realise what is happening, and if you do, it's usually far too late to do anything about it. Being parked up for a long time was likely the culprit here, as there was no amount of fresh-air passing through the vents from (a lack of) forward speed of the car.
